Health Questionnaire to Get Health Insurance Quote

Generally, insurance companies will have you fill out a health questionnaire and/or go in for a physical before considering your application for health insurance. Here are some questions you may be asked, in order to have the answers on hand.

  • Are you applying for individual health coverage within 90 days of finishing your COBRA coverage?
  • Have you had at least 24 months of previous insurance coverage before applying?
  • Are you, or do you plan to add coverage to your existing policy for a newborn or adopted child?
  • Do you plan on extending your family within the next year?
  • Is any family member pregnant or an expectant mother or father?
  • Do you have any pre-existing conditions, such as diabetes, cancer, high blood pressure or high cholesterol?
  • Do you have any family history of heart disease?
  • Do you currently use, or have ever used any form of tobacco?
  • Within the last 5 years, has any applicant been advised to take medicine for depression or any other physiological issue?
  • Does the applicant have any indications or symptoms of chest pain, paralysis, arthritis, STDs, tumors, cysts, mental, emotional or other behavioral disorder?

These are just a few questions you will find on most health insurance questionnaires. It is always a good idea to have this information handy when requesting quotes so that your time is not wasted trying to figure out answers to certain questions.
